Michael R Polk

Michael Randolph Polk, age 48, passed away at Hospice of Citrus County and the Nature Coast, March 21, 2017.  Mr. Polk was born in Starke, Florida on January 25, 1969 to Ronald Edward Polk and Marjorie Bass-Polk.

Preceded in death of his father Ronald Polk and brother John Steven Polk.
